Inc. offers both a Point of Service plan and a series of HMO's with
various co-pays. The plans can be purchased with or without a
prescription benefit. Chamber members can join the Aetna plans at any
time and change their plan on their one year anniversary. To be
eligible, a person must work a minimum of thirty hours per week in his or her own business.

Assurant Employee Benefits
Dental now available for sole proprietors.
Provides freedom to choose dentist (including specialists) or PPO option,
12-Month rate guarantee and no referrals required. Benefits paid after
deductible met, up to the annual maximum. Claim payments may be made to you
or your dentist, unless benefits have been assigned to the provider. For a

Enrollment deadline is the 20th of each month for the 1st of the following month.
Groups of 2 to 9 employees are eligible. 100% enrollment is required.

of Massachusetts' Preferred One Plan is available for sole
proprietors. When you receive care from network (panel) dentists, there is
no co-payment for preventive and diagnostic services like cleanings and

Community Health Plan Includes dental coverage and "direct"
